New service, supported by Nebraska’s Rural Health Care Transformation Program, positions Providence as a hub for remote healthcare across northeast Nebraska.
WAYNE, Neb. — Providence Medical Center (PMC) today announced the launch of a Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) program that extends the hospital’s care beyond its walls and into patients’ homes. The new service is supported through Nebraska’s Rural Health Care Transformation Program (RHTP) and establishes Providence as a regional hub for remote healthcare serving Wayne and the surrounding communities.
Remote Patient Monitoring uses simple, easy-to-use home devices — such as blood pressure cuffs, glucose meters, pulse oximeters, and scales — to capture a patient’s health readings and send them securely to a Providence care team. The team reviews the readings, watches for anything outside a safe range, and escalates concerns to the patient’s provider. Through Chronic Care Management, a dedicated nurse Care Coordinator checks in with patients between visits to answer questions, support medications, and keep care on track.
The program is designed for patients living with ongoing conditions that benefit from steady, day-to-day attention — including high blood pressure, diabetes, heart conditions, COPD, and chronic kidney disease. By catching small changes early, RPM aims to help patients stay healthier at home, avoid unnecessary hospital visits, and remain independent.

The service is funded in part through the RHTP, a historic federal investment created to strengthen health care in rural communities. Nebraska is receiving approximately $218.5 million in the program’s first year as part of a broader multi-year commitment. Providence has been awarded $311,701.87 to support the rollout of remote care, technology, and staff training. Because many area residents receive primary care from independent physicians, Providence is coordinating closely with local providers so patients experience a seamless, connected program. Providence will serve as the central hub — providing the monitoring technology, the clinical team, and the coordination — while patients continue to see the providers they know and trust.
