[Wayne, NE – July, 2026] – Providence Medical Center announced today it has been selected by the Nebraska Department of Health and Human Services (DHHS) to receive a grant under the state’s Rural Health Transformation Program (RHTP). This critical funding will directly support Providence Medical Center’s efforts to improve healthcare delivery, accessibility, and local resources across Northeast Nebraska.
The RHTP initiative, backed by a federal allocation managed through Nebraska DHHS, aims to stabilize rural healthcare infrastructure, accelerate workforce retention, and launch innovative care delivery models. [1, 2]

With this grant award, Providence Medical Center will implement the following key initiative over the next fiscal cycle: [1]
- Remote Patient Monitoring: PMC will implement a Remote Patient Monitoring program that enables care teams to monitor patients’ health remotely, improve management of chronic conditions, identify concerns earlier, reduce avoidable hospital visits, and enhance access to quality healthcare while allowing patients to remain safely at home.
