Request Medical Records

At Providence Medical Center, we’ve partnered with VRC to make requesting your medical records simple and secure.

You can submit your request directly through VRC’s online portal.

What You’ll Need

• Your full name and date of birth
• Contact information
• The type of records you need (labs, imaging, visit notes, billing, etc.)
• Dates of service or time period
• Photo ID for verification

Delivery & Processing

• You’ll be able to choose how you want to receive your records — by mail, fax, or
  secure email.
• Requests are usually completed in 3–7 business days, and VRC will contact you
  directly if additional information or fees apply.

• Can someone else request my records?
  Yes — they’ll need signed authorization and a photo ID.
• How will I know when my request is ready?
  VRC will send you a text or email update when your records are ready.

  Copy fees may apply based on state and federal rules. You’ll be notified
  before any charges are made.
