Remote Patient Monitoring (RPM) lets you take simple readings — like blood pressure, blood sugar, weight, or oxygen — from the comfort of home. Your numbers are sent securely to a Providence Medical Center care team that watches for anything concerning and alerts your doctor when it matters. It’s an extra layer of care between your regular visits.
RPM is especially valuable for ongoing conditions that benefit from steady, day-to-day attention.

Using a home device, record your blood pressure, weight, oxygen level, or other measureements.
Your readings are securely reviewed by your Providence Medical Center care Team.
If a reading is concerning, your provider is notified so care can be adjusted quickly.

Through Chronic Care Management (CCM), a dedicated Care Coordinator — a nurse — stays in touch between doctor visits. They’re a familiar voice who knows your story.
Remote Patient Monitoring and Chronic Care Management are covered by most insurance plans, including Medicare. Depending on your plan, a copay may apply. Coverage and out-of-pocket costs vary, so we recommend checking with your insurance carrier for your specific eligibility. Our team is happy to help you understand your options.
