Remote Patient Monitoring

New Service Under the Rural Health Care Transformation Program

Your health, watched over — right from home.

Providence Medical Center now offers Remote Patient Monitoring: simple home devices and a caring team that keeps an eye on your numbers between visits.

Care that comes home to you

Remote Patient Monitoring (RPM) lets you take simple readings — like blood pressure, blood sugar, weight, or oxygen — from the comfort of home. Your numbers are sent securely to a Providence Medical Center care team that watches for anything concerning and alerts your doctor when it matters. It’s an extra layer of care between your regular visits.

Helpful for the conditions we see most

RPM is especially valuable for ongoing conditions that benefit from steady, day-to-day attention.

High Blood Pressure

Daily readings help keep hypertension in a safe range.

Diabetes

Track blood sugar and spot trends before they become problems.

Heart Conditions

Support for heart health and recovery, monitored closely.

COPD & Breathing

Oxygen checks help you breathe easier with confidence.

Kidney Health

Keep an eye on weight and pressure to protect kidney function.

How it works

Simple for you. Reassuring for your family.

Easy devices at home

We provide simple, ready-to-use devices. Take your readings in seconds — no technical skills needed.

Your team is watching

Your readings go securely to a Providence Medical Center care team that reviews them and looks for anything out of range.

Escalated to your doctor

If something needs attention, the team acts quickly and notifies your provider — so small issues stay small.

A nurse who checks in on you

Through Chronic Care Management (CCM), a dedicated Care Coordinator — a nurse — stays in touch between doctor visits. They’re a familiar voice who knows your story.

  • Regular check-in calls to see how you’re doing
  • Help with medications, questions, and appointments
  • A direct line to your care team when something changes
  • Peace of mind that someone is paying attention

Real peace of mind

Someone is watching

Your readings are reviewed by real people, not left in a drawer.

Fast escalation

Concerning numbers reach your doctor quickly — before they become emergencies.

Stay home, stay independent

Fewer trips to the clinic, more confidence living life on your terms.

Care that knows you

Delivered by Providence, close to home, by people in your community.

Family reassurance

Loved ones rest easier knowing a team is keeping watch.

Part of a bigger mission

Made possible through Nebraska’s Rural Health Care Transformation Program.

Is it covered?
